Receipts are issued once a month, and clients should be made aware of this. If a client wishes to receive receipts, please provide the details below, including the dates for which receipts are needed.

Client Risk Alert

Use this form if a client discloses suicide risk, self-harm, a child in danger, or another serious safeguarding concern and you need a call back from the clinic team.

Important: Do not include the client’s name or any identifying details in this form. Keep full clinical details in your notes and supervision.
  • Follow your professional code of ethics and duty of care.
  • Use emergency services (e.g. 999) if there is immediate danger.
  • Record a full clinical note and bring to supervision.
